By checking this box, I confirm that I have the right to register the child of whom I am the parent or legal guardian (“Child”) in the Jeux de Montréal’s virtual Passe-Sports challenge (the “Program”) organized by Sports Montréal, Inc. (“Sports”) from April 7 to June 30, 2021, that I want the Child to participate in one or all of the Program’s activities and that I can upload videos of the Child completing the challenges to the Program’s online platform (“Platform”).
I represent and warrant that the Child is of the age required to participate and that I and the Child are physically able to perform the challenges and have no pre-existing conditions that would limit us from participating in physical and sports activities and that would prevent our participation in the Program. I understand that it is my responsibility to consult a physician prior to the Child’s and my participation in the Program. I further understand that it is my responsibility to ensure that the physical location where the Child engages in any of the Program’s activities is suitable for physical activity and free of any obstructions and that the activity is carried out in a safe and prudent manner.
I agree to upload the videos of the Child completing the challenges on the secure Platform so that they can participate in the contest, provided that they are a resident of the Ville de Montréal or of one of the cities in the Greater Montréal area. I understand that any uploading of false videos or of videos depicting inappropriate or obviously unsafe behaviour, or any attempt to cheat will result in the withdrawal of the right to participate in the Program. I further understand that the site will be secure and that the Child’s videos will only be used to validate the challenge by Sports Montréal’s Jeux de Montréal organizing committee. No other distribution will be allowed without my consent and the videos will be deleted at the end of the Program. I understand that once the videos are uploaded, I will not be able to retrieve them. However, I could ask that the videos be deleted at any time before the end of the Program by writing to [email protected].
I assume full responsibility for any and all risks, injuries or damages, known or unknown, that I and the Child may incur as a result of our participation in the Program, and I expressly release Sports Montréal and all parties associated with the Program, including, but not limited to, the Ville de Montréal, the organizing sports associations, sponsors and partners, suppliers, athletes, ambassadors and the Program’s spokesperson and coaches, from any and all recourses, proceedings, claims and causes of action of any nature whatsoever arising out of or in any way connected with the Child’s or my participation in the Program.
I agree to receive email communications in connection with the Program.
